Getting Started

Nutriquity is in-network with the following insurance providers:

  • Blue Cross Blue Shield PPO

  • Bluechoice PPO

  • UnitedHealthcare

  • Cigna

  • Medicare Part B (diabetes and CKD only)

We can run a preliminary benefits check to find out if your insurance covers nutrition counseling - FREE INFO SESSION

 

You should also confirm your benefits before your initial session  - CHECK YOUR BENEFITS.   Then you will:

  1. Book an initial 60 minute nutrition counseling consultation directly with Registered Dietitian Michael Glab ​or click HERE to book online.

    • Cancellations must be made at least 24 hours in advance to avoid $25 cancellation fee

    • Appointments can also be rescheduled with at least 24 hours advance notice

  2. A link will be emailed to you to complete a client profile in our HIPAA-compliant virtual clinic and you will do the following:

    • Complete the Nutriquity New Client Assessment 

    • E-sign our terms & conditions and consent for telehealth

    • Put a credit card or HSA on file (for copays, coinsurance, any uncovered services or cancel fees)

    • Upload a picture of the front and back of your insurance card (or bring to the initial session) 

    • Upload a picture of your ID or driver's license (or bring to the initial session) 

  3.  At the initial consultation we will

    • Review assessment form

    • Obtain baseline weight and other desired measurements (hip, waist, etc) 

    • Set client short and long-term goals

    • Develop a dynamic nutrition plan 

    • Provide daily calorie and protein targets  (if desired)

    • Provide supporting materials (educational handouts and/or recipes)

    • Set up follow session(s) as desired.

  4.  Cancellation Fees ​​

    • Cancellation of an appointment with less than 24 hours notice is subject to $25 fee. 

    • No call/no show policy - If a client does not show up for a scheduled appointment the client will be charged 50% of the insurance billable hourly rate or 100% of the out-of-pocket rate.  This fee is non-refundable and cannot be applied as credit toward a future session.  

 

No Insurance, no problem

Many clients are not able to use insurance benefits for a variety of reasons.  We offer affordable rates to ensure you receive the highest quality care you deserve.
