New Client Onboarding

Getting Started

Nutriquity is in-network with the following insurance providers:

  • Blue Cross Blue Shield PPO

  • Bluechoice PPO

  • Aetna

  • UnitedHealthcare

  • Cigna

  • Medicare Part B (diabetes and CKD only)

  • Referrals are needed for Aetna, United and Medicare 

    • Fax referrals to 708-742-7868​

You should confirm your benefits before your initial session  - CHECK YOUR BENEFITS.

  1. Book an Info Session with Registered Dietitian Michael Glab ​to learn more about how professional nutrition counseling can help you achieve your goals.​

  2. If you'd like to move forward, a link will be emailed to you to complete our intake forms in our HIPAA-secure platform.​

    • Nutriquity New Client Assessment 

    • E-sign our terms & conditions and consent for telehealth

    • Put a credit card or HSA on file (for copays, coinsurance, any uncovered services or cancel fees)

    • Upload a picture of the front and back of your insurance card (or bring to the initial session) 

    • Upload a picture of your ID or driver's license (or bring to the initial session) â€‹

  3. Once all intake forms are complete, you can book your Initial consultation directly with Michael or through our website. ​

  4.  At the initial consultation we will​

    • Review your assessment form

    • Collaborate on a dynamic nutrition plan â€‹

    • Provide daily calorie and protein targets if desired​

    • Set realistic goals 

    • Provide supporting materials like recipes and handouts ​​​

    • Set up follow session(s) as desired.

  5.  Cancellation Fees â€‹â€‹â€‹

    • Rescheduling of appointments with less than 24 hours notice may be subject to $40 rescheduling charge for the first incident. All further incident's will result in a $65 charge. â€‹

    • No Call, No Show policy -  A "No Call, No Show" fee will go into effect after 10 minutes have elapsed from the scheduled appointment time without The Client being present or without The Client providing prior communication (email, phone call or text) of their absence.  A "No Call, No Show" fee of 100% of the insurance billable hourly rate or 100% of the out-of-pocket rate. This fee is non-refundable and cannot be applied as credit toward a future session for both insurance or cash pay Clients.

  6. Nutriquity LLC is a teaching practice

    • Nutriquity has aligned with UIC to provide dietetic interns real world experience working with clients.  We may have interns sitting in on sessions providing assistance and further insights for our clients.  Please let us know in advance if you are not comfortable with this.​

No Insurance, no problem

Many clients are not able to use insurance benefits for a variety of reasons.  We offer affordable rates to ensure you receive the highest quality care you deserve.
